Hi hmeah,
You wouldn’t normally need to do this yourself. Your accountant would normally either form the company for you, or point you towards one of the many filing agents who will do this for you. It only costs about £35 to form a limited company through an agent and they will ( a) get it done right and (b) get it done within a few hours as opposed to a couple of weeks and possible wrongly if you are doing a paper formation direct with companies house which it sounds like you are trying to do.
The wider question however is whether or not this is the right business structure for you? It may well be more appropriate to start trading as a partnership/sole trader rather than a limited company from the off due to the increased level of paperwork and costs of being a company. You should be able to get some help on this as part of a free consultation with most accountants.
Hope this helps.

hmeah,
Stop DON'T DO IT. Me and the wife have had a sandwich bar for 3 years now and went down the Ltd Company VAT Reg route. You'll pay your accountant £200 per couple of months and the Customs people will send you a bill for a grand a quarter. This happened to me - worst, worst mistake ever.
For the love of God don't even consider this seriously.....

What are you paying your accountant £200 for every couple of months? As per post in other thread seems a bit steep to me!
